  • Step:1 Go to official site for the download passport application form.
  • Step:2 Now select option for fresh or Re Issue passport.
  • Step:3 The form will immediately download to your pcs.
  • Step:4 Application can fill up the form online as well as offline also.
  • Step:5 Once you fill up the form go to passport seva portal and complete all the details.

Online Passport Form Filling

Here are quick guidelines for you to online passport filling.
  • Applying for Fresh or Re-issue of passport (Select the option from drop down accordingly)
  • Select the button of Normal/Tatkal in type of applications.
  • Select type of passport booklet.
  • All your personal details are required. Like name, address, marital status, spouse's details, father's name and date of birth.
  • If you are holding a PAN Card and Aadhar Card, mention the numbers in the form .
  • If you are renewing for passport, mention your expired passport number.
  • After filling up the entire form, click on Submit.
  • You can view your submitted application. Right from the same page, you can proceed to the next step. Give a click on button of Pay and Schedule Appointment. Select nearest PSK to your residence.
Online Form Submission
  • Step 1: Register through the Passport Seva Online Portal. (Click on "Register Now" link on the Home Page).
  • Step 2: Login to the Passport Seva Online Portal with the registered Login Id.
  • Step 3: Click "Apply for Fresh Passport/Re-issue of Passport" link.
  • Step 4: Fill in the required details in the form and submit.
  • Step 5: Click the "Pay and Schedule Appointment" link on the "View Saved/Submitted Applications" screen to schedule an appointment.
    • Online Payment has been made mandatory for booking appointments at all PSK/POPSK/PO.
    • Online Payment can be made using any one of the following modes:
    • Credit/Debit Card (Master Card and Visa)
    • Internet Banking (State Bank of India (SBI) Associate Banks and Other Banks)
    • SBI Bank Challan
  • Step 6: Click the "Print Application Receipt" link to print the application receipt containing Application Reference Number (ARN)/Appointment Number.
  • Note: Carrying printout of Application Receipt is no longer required. An SMS with your appointment details is also accepted as proof of appointment during your visit to Passport Office.
  • Step 7: Visit the Passport Seva Kendra (PSK)/Regional Passport Office (RPO) where appointment has been booked, along with original documents.
